Housing Consultation

Housing Consultation Services helps a person who does not have Medical Assistance (MA) case management develop a person-centered plan that addresses their needs, wants, and goals for living in the community.

Housing Transition

Services that assist a person in planning for, finding, and moving into housing. We assist a person in identifying affordable housing options that meet their needs, goals, and budget.

Housing Sustaining

Our team connects the person we serve with local organizations and agencies that provide housing-related services, financial assistance, mental health support, employment resources, and other relevant assistance programs.

Who Can Get Services

  • People on Medical Assistance (MA)
  • Be 18 years old or older
  • People with documented disabilities or disabling conditions
  • People who are homeless or at risk of becoming homeless.
  • Adults who are 65 years of age or older who are homeless
  • or at risk of becoming homeless.
  • Have a qualifying assessment.
