Our Housing Consultation Services provide the personalized planning you need to move confidently into safe, stable housing. We work closely with you to define your goals, complete eligibility paperwork, connect you to benefits, and coordinate care—ensuring every step reflects your needs, preferences, and long-term stability.


Housing Consultation is the first step in the HSS program and is intended for individuals who do not already have a waiver case manager (e.g., CADI, BI, or DD waivers). It is a planning service that ensures the individual has a clearly defined, person-centered housing plan to support their transition into stable, independent living.

What We Include in
Our Housing Consultation

Development of a Person-Centered Housing Plan

We help each client articulate their housing goals, support needs, preferred locations, accessibility requirements, and cultural or personal preferences. This includes long-term stability objectives and how they define a safe, secure home.

HSS Eligibility Paperwork

We guide clients through completing all required documentation to determine eligibility for Housing Stabilization Services, ensuring they meet state criteria and nothing is overlooked.

Informed Provider Choice

Clients are given information and guidance to make informed choices about their transition and sustaining providers. This empowers them to select the most appropriate support agency for the next stage

Coordination of Care

We collaborate with any existing providers (e.g., healthcare, mental health, case management, employment counselors) to ensure the plan complements the individual’s broader wellness and life goals.

Cultural and Language Consideration

Plans are tailored to meet each client’s cultural, language, and religious preferences, which are essential to long-term housing satisfaction and stability.
