Housing Consultation Services helps a person who does not have Medical Assistance (MA) case management develop a person-centered plan that addresses their needs, wants, and goals for living in the community.

• We will assist in developing a housing focus person-centered plan.– Assist the person served in completing forms required for Housing Stabilization Services eligibility.
• We Support the person in identifying their needs and wants in housing, including cultural requirements and preferences.
• Help to identify and apply for benefits
•  Assist individuals in making an informed choice in their housing transition or sustaining services provider.
  
• Coordinating with other service providers already working with the person.
