Housing Stabilization Services (HSS)
This service is a medical assistance benefit that assists individuals with disabilities, including mental illness, seniors, and individuals at risk of homelessness, to find and keep housing.
As aHousing Stabilization Services(HSS) provider,Our goal is tosupport an individual’s transition into housing and maintain housing to avoid future periods of homelessness or institutionalization. We work with the people we serve every step of the way to overcome barriers that may stand in the way.
HSS comprises of three services:Housing Consultation, Housing Transition, and Sustaining:

Services We Provide
Eligibility:
- People on Medical Assistance (MA)
- Be 18 years old or older
- People with documented disabilities or disabling conditions
- People who are homeless or at risk of becoming homeless.
- Adults who are 65 years of age or older who are homeless or at risk of becoming homeless.
- Have a qualifying assessment.
