Housing Consultation is the first step in the HSS program and is intended for individuals who do not already have a waiver case manager (e.g., CADI, BI, or DD waivers). It is a planning service that ensures the individual has a clearly defined, person-centered housing plan to support their transition into stable, independent living.
This service helps individuals actively plan for, search for, and move into a home that aligns with their documented needs and preferences. It supports every aspect of the relocation process and removes common barriers to independent living.
Once a client is housed, sustaining services are designed to keep them stably housed. These services provide ongoing support, education, and problem-solving to avoid eviction, conflict, or destabilization.
HCBS offers essential support for seniors and individuals with disabilities who prefer to remain in their homes rather than move into institutional settings.
