What We Include in
Our Housing Transition

Affordable Housing Identification

We help locate available units that align with the client’s financial resources, preferences (e.g., location, building type), and support needs.

Application & Paperwork Support

From filling out rental applications to gathering income documentation, we ensure nothing stands in the way of a timely move.

Accessibility & Safety

We evaluate prospective units to confirm they meet safety standards and accommodate physical, mental, or sensory disabilities. This may include ensuring the presence of ramps, grab bars, or quiet environments.

Landlord Communication & Advocacy

We act as a liaison between the client and landlord/property manager, explaining the HSS program and advocating for reasonable accommodations when needed.

Move-In Logistics

We help coordinate inspections, sign lease agreements, arrange deposits, and schedule move-ins, ensuring a smooth and supported transition into the new home.

Utility Setup and Coordination

We assist with initiating essential services such as electricity, water, heat, internet, and garbage disposal to ensure the client has a fully functioning home environment.

Benefit Application Assistance

If clients are not yet receiving benefits (e.g., SSI, SSDI, housing vouchers), we help them apply for the right programs to support their housing stability.
