Housing Stabilization Services (HSS)
This service is a medical assistance benefit that assists individuals with disabilities, including mental illness, seniors, and individuals at risk of homelessness, to find and keep housing.
As a Housing Stabilization Services(HSS) provider, Our goal is to support an individual’s transition into housing and maintain housing to avoid future periods of homelessness or institutionalization. We work with the people we serve every step of the way to overcome barriers that may stand in the way.
HSS comprises of three services: Housing Consultation, Housing Transition, and Sustaining:
Housing Consultation
Housing Consultation Services helps a person who does not have Medical Assistance (MA) case management develop a person-centered plan that addresses their needs, wants, and goals for living in the community.
- We will assist in developing a housing focus person-centered plan.– Assist the person served in completing forms required for Housing Stabilization Services eligibility.
- We Support the person in identifying their needs and wants in housing, including cultural requirements and preferences.
- Help to identify and apply for benefits
- Assist individuals in making an informed choice in their housing transition or sustaining services provider.
- Coordinating with other service providers already working with the person.
Housing Transition
Services that assist a person in planning for, finding, and moving into housing.
- We assist a person in identifying affordable housing options that meet their needs, goals, and budget.
- Assist with the housing search, application, and documentation process.
- Ensuring the new living arrangement is safe for the person and ready for move-in.
- Ensuring adaptive accommodations are in-place.
- Advocating on a person’s behalf to potential landlords.
Sustaining
Services that support a person to keep their housing after they have moved in.
- Our team connects the person we serve with local organizations and agencies that provide housing-related services, financial assistance, mental health support, employment resources, and other relevant assistance programs.
- Our housing Case Manager will check in with the person served regularly to ensure their housing situation remains stable and address any emerging issues.
- Provide education and training on cultural sensitivity to develop a good understanding with prospective landlords, property managers, neighbors, and roommates.
- Assist in understanding and applying for housing-related benefits to retain housing.
- Navigating Economic Assistance
- Identify strategies to manage a budget effectively.
Home And Community-Based Services (HCBS)
Home and Community-Based Services (HCBS) are provided to people with disabilities and those over 65.

Who Can Get Services
- People on Medical Assistance (MA)
- Be 18 years old or older
- People with documented disabilities or disabling conditions
- People who are homeless or at risk of becoming homeless.
- Adults who are 65 years of age or older who are homeless or at risk of becoming homeless.
- Have a qualifying assessment.
